How to create an external form

Building the form

An external form is built in exactly the same way as a stage or governance report.

When building your form(s) it is important to consider whether there will be a requirement for the data in the external form to flow through into subsequent stages in the site i.e. could a submitted idea become a project? If it could, you must ensure that the same fields are being used in the external form for the same purpose in each stage.

What we need to know

Once you are happy with your external form configuration, you will need to contact Verto Support with answers to the following information:

  • Do you require a heading or logo on your external form? This is the banner which will sit at the top of the external form screen and can contain an organisational logo/image or heading text.
  • Do you require a welcome message to be displayed on screen? This will be visible on the screen after the individual has logged in i.e., Welcome to the Verto ideas and suggestions portal. Please complete the form as fully as possible. If you need to, you can pause your completion and return later. 
  • What should the email subject line state? This is the email which is generated when the individual adds a new form .i.e. Your Idea Has Been Added to Verto
  • What should the email include? This is the text contained in the email which is generated when the individual adds a new form. It will contain a link to their form so they can return to complete it later i.e. Hello. Your idea / suggestion has been successfully added to Verto. If you need to return to the form at any point you can locate it here. Thank you! 
  • Do you require a 'completed' message? This will display on the last screen confirming that the form has been fully completed and can be submitted i.e. If you are happy with all the information you have entered, just hit 'submit' and you're done! Alternatively, click on 'Previous' to review your answers. 
  • Do you require a 'thank you' message? This will appear at the end of the process.

External form URL

Once your external form is ready, a member of the support team will provide you with your unique URL. This can then be shared as needed on intranet sites, websites, SharePoint etc. 

Remember

  • External forms are built in the same way as a standard stage or governance report.
  • You can have multiple external forms.
  • Submitted forms will feed directly into Verto and can follow the same approval workflows as needed.
